At Commonwealth Prosthodontics we are proud to be a part of a team whose primary mission is to deliver the finest and most comprehensive dental care today. It is our belief that financial considerations should not be an obstacle to obtaining the best care available to you. Realizing that there are different needs in fulfilling financial obligations, we provide multiple payment options to assist you with your oral healthcare investment. Our goal is to make the care you need and deserve accessible, regardless of your budget.

Once a treatment option is selected, payment arrangements are discussed with our financial coordinator. We are a non-participating, fee-for-service practice. Payment is expected at the time of service. For your convenience, we accept Visa, MasterCard, and Discover.

If you have insurance, pre-determinations are filed promptly, and as a service to the patient, we will process your claim.

PAYMENT OPTIONS:

Payment in Full

A bookkeeping courtesy of 3% is available for payments made in full by check prior to the start of treatment.

50/50

Fifty percent of your total treatment cost is due at or before the beginning of your treatment. Your remaining balance is due at the completion of your treatment.

Office Installment Plan (Within estimated length of treatment)

An initial payment is due at or before the first treatment appointment, with subsequent payments due at the first of each month thereafter until account is paid.

Lowest Monthly Payment Plan:

  • Prepayments can be made anytime without penalty
  • 0% interest financing for up to 12 months.

Payments are considered past due at 30 days from the payment due date are subject to a late fee and, if necessary collection fees. Returned checks, for any reason, are subject to a $50.00 returned check fee. Charges may also be made for broken appointments cancelled without the required 24 hours notice. Six months after completion of treatment, an office visit fee will be charged.
